The Saint Alphonsus Geriatrics Program provides a comprehensive and compassionate approach to seniors who are dealing with cognitive impairments and chronic conditions. Our team of healthcare professionals, including a physician, pharmacist, and social worker, work closely with the patient and their family to provide the tools and education to help the patient age gracefully and prolong their independence as long as possible.
If you think you or a family member could benefit from our program, a primary care provider can arrange a referral into our clinic. Alternatively, you can call any of our clinics to discuss scheduling a consultation.
