Saint Alphonsus Geriatrics Program

The Saint Alphonsus Geriatrics Program provides caring, complete healthcare for older adults who are living with memory problems, chronic illnesses, or other age-related health concerns.

Our team includes a doctor, pharmacist, and social worker who work closely with patients and their families. Together, we provide education, support, and treatment to help older adults stay healthy, maintain their independence, and enjoy the best possible quality of life.

Our team can help with:

  • Evaluating common health issues in older adults, including:
    • Memory loss and cognitive impairment
    • Confusion or delirium
    • Depression
    • Weakness and frailty
    • Falls
    • Bladder control problems
    • Poor nutrition and weight loss
  • Diagnosing memory and thinking problems through specialized memory testing
  • Treating chronic conditions, including:
    • Dementia
    • Alzheimer’s disease
    • Parkinson’s disease
    • Osteoporosis
    • Insomnia (sleep problems)
    • Heart failure
    • COPD (chronic lung disease)
    • Chronic kidney disease
    • Unexplained weight loss
  • Reviewing medications to make sure they are safe, effective, and working well together
  • Helping patients and families plan for future healthcare needs
  • Assisting with transitions to higher levels of care, such as assisted living or memory care communities
  • Providing support, education, and resources for family members and caregivers
  • Coordinating care with primary care providers and medical specialists
  • Offering spiritual counseling and support

Falls are a major cause of injuries and death for adults age 65 and older, but many falls can be prevented.

Saint Alphonsus offers a free 10-week Fit and Fall Proof class designed to help older adults improve strength, balance, and confidence to reduce their risk of falling.
