Saint Alphonsus Anticoagulation Clinic helps people who take warfarin (a blood-thinning medicine) stay safe and on the right dose.

At the clinic, trained pharmacists:

  • Check your blood level (called INR) using a quick finger stick
  • Adjust your medicine dose right away if needed

Most of the time:

  • You won’t need a blood draw from your arm
  • You won’t have to wait for a phone call to know your dose

If the pharmacist has any questions, they will talk with your regular doctor.

Saint Alphonsus has been providing this service to patients and doctors since 2005.

Using a patient-centered approach to warfarin therapy, our focus is to educate and empower patients taking warfarin, to keep providers well-informed about their patient’s care, and to provide helpful, compassionate care to each patient.

Patients must have a referral from a provider to schedule an appointment at one of our Anticoagulation Clinics. All visits are by appointment only.

Clinics are located in Boise, Eagle, Meridian, Nampa, and Ontario, OR.
