We know that lifestyle modification is the most effective means to prevent cardiovascular disease and is often underutilized. Our Preventive Lifestyle Clinic aims to treat heart disease and stroke risk factors early in the process to help prevent a heart attack or stroke.

Is the Preventive Lifestyle Clinic for You?

A preventive medicine patient is someone with one or more of the following criteria:

  • Heart Disease
  • Metabolic Syndrome

What is Preventive Medicine?

Preventive medicine is a medical specialty focused on healthy lifestyle choices of the patient. By living a healthy lifestyle, we help prevent severe medical conditions such as heart disease and stroke.

The Preventive Lifestyle Clinic focuses on the whole patient and the various factors that impact their health.

How does the Preventive Lifestyle Clinic work?

Patients undergo a twelve-step program to reduce cardiovascular disease risk. Each patient has an Individualized plan created for them that includes lifestyle and medical management of:

  • Nutrition/Diet
  • Exercise

  • Waist circumference/Weight
  • Cholesterol
  • Tobacco use
  • Blood pressure
  • Stress
  • Sleep
  • Diabetes
  • Dental health
  • Alcohol use

Patients have an extensively planned visit with an advanced practice provider. Patients are given the knowledge and tools to prevent chronic disease. 

Patients will meet with a dietitian as well as an exercise physiologist.  Goals are created with each patient and follow up is provided as necessary. 

Once they achieve all 12 steps, they graduate from the clinic.
