What is Prediabetes?
Prediabetes is blood glucose (sugar) level that is higher than normal but not high enough to be diagnosed as diabetes. One in three American adults has prediabetes, and do not know they have it. If you have prediabetes and do not lose weight or do moderate physical activity, you can develop type 2 diabetes within 5 years. Type 2 Diabetes is a serious condition. It can lead to health issues such as heart attack; stroke; blindness; kidney failure; or loss of toes, feet or legs.
About Prevent T2 Program
Prevent T2 is part of the National Diabetes Prevention Program, led by the Center for Disease Control and Prevention (CDC). It is proven to help people with prediabetes prevent or delay development of type 2 diabetes. The research-based program can reduce your risk of developing type 2 diabetes by up to 58% (71% for people over 60 years old). View the Diabetes Prevention Program Research Study.
Program Goals
The Saint Alphonsus Prevent T2 Program is designed to help you meet the following goals:
- Reduce your body weight by 5% to 7%
- Increase your physical activity to 150 minutes each week
- Increase your coping skills
What to Expect
Prevent T2 group meet for a year – weekly for the first 4 months, then 1-2 times a month for the remainder of the program. During each session, your lifestyle coach will teach a lesson and lead a group discussion.
Examples include:
- Healthy eating
- Adding physical activity to your life
- Managing stress
- Stay motivated and solving problems that can get in the way of health changes
Your Prevent T2 Lifestyle Coach
Prevent T2 lifestyle coaches have the experience and training to help you reach your goals. Your lifestyle coach will help you:
- Learn the facts about healthy eating and physical activity and explain how these behaviors will help reduce your risk for type 2 diabetes
- Set and meet your goals
- Build relationships with other participants
- Work as a group to meet challenges
- Work with you on any barriers to accessing care and connect you with community resources to help address your social needs.
Get Started
1. Take the risk test to find out if you are at risk for prediabetes and type 2 diabetes.
2. If you are at high risk, talk to your doctor about getting a blood sugar test (A1C).
3. If you have prediabetes, complete the interest form to join a Prevent T2 program.
Participant Eligibility
- 18 years or older
- A body mass index (BMI) of 25 or higher (23 or higher for people of Asian heritage)
- Have prediabetes or are at high-risk for type 2 diabetes:
- Blood test result in the prediabetes range or
- Ever had diabetes while pregnant (gestational diabetes) or
- High-risk score on the prediabetes risk test
People who are pregnant or have been diagnosed with type 1 or type 2 diabetes are not eligible.
