Our team is dedicated to building strong relationships with clinics, hospitals, and community healthcare workers—including EMS—through timely information, clear communication, and a shared commitment to high quality, coordinated patient care.

As trusted liaisons, we work closely with clinics, regional hospitals, and EMS partners to identify opportunities, address concerns, and remove barriers to care. Through advocacy, education, and hands on support, we help our partners navigate SAHS resources while fostering collaboration across the care continuum.

How We Support Our Community Healthcare Partners

Our team offers comprehensive support designed to make working with Saint Alphonsus seamless and efficient:

• A single, trusted point of contact connecting clinics, hospitals, community healthcare workers, and EMS partners with SAHS leadership and service lines
• Information on new programs, clinical services, specialists, technologies, educational opportunities, and events
• Assistance with referrals and transfers, including coordination with specific clinical service areas to resolve issues efficiently
• Access support for EpicCare Link to view medical records and monitor referral status
• Coordination of in person and virtual meetings with specialists to enhance communication and care coordination
• Advocacy to ensure partner feedback is heard and addressed
• Support for care coordination initiatives, including collaboration with EMS to improve transfer protocols and awareness of system resources
