Brandie Kramer, Program Specialist, Senior Care
As the Senior Care Program Specialist, Brandie Kramer coordinates and develops the Senior Care program strategies and establishes senior health partner's within each community from Baker City, Ontario, Nampa and Boise. We will identify opportunities to improve the patient and family experience of care and to improve the efficiency and effectiveness by using community and regional resources. Brandie also creates community driven care management collaborative and partnerships in the identification of community educational needs for seniors and will provide information and referrals to services and resources relating to older adult issues within each community, as well as with outside agencies.
