Community Health Needs Assessment Overview

The Community Health Needs Assessment (CHNA) helps Saint Alphonsus use its resources wisely. It allows us to focus our time, effort, and funding on the areas where they are needed most and where we can make the biggest impact.
A Community Health Needs Assessment helps us:
- Understand the health needs and strengths of our communities
- Identify and support people who may be most at risk
- Build stronger partnerships and work together with community groups
- Plan, improve, and measure our outreach programs
