Your rights as a patient in the Rehabilitation Program at SARMC are:

  • To receive kind and considerate care in a safe setting.
  • To receive care regardless of race, creed, sex, national origin, or sources of payment.
  • To receive respect for your personal values and beliefs.
  • To receive complete information about your condition.
  • To participate in the development and implementation of your care plan.
  • To have access to information contained in your clinical records.
  • To request or refuse treatment.
  • To know the names and roles of clinic staff.
  • To be told about clinic rules that apply to your conduct.
  • To be free from restraints or seclusion unless necessary for your well-being.
  • To receive care that protects your privacy.
  • To have your clinical records kept confidential.
  • To review your medical records and receive answers to your questions.
  • To have an interpreter or communication aids as needed.
  • To have explanation if the need to transfer to another provider is warranted.
  • To know if the clinic has relationships with outside organizations that may affect your care or treatment.
  • To ask questions and receive information about your bill for treatment.
  • To say yes or no if asked to take part in research.
  • To share concerns with the clinical manager about your environment or the care you receive.
  • To file a grievance regarding your quality of care.
  • To be given choices when you no longer need to be in the hospital.
  • To have someone represent you legally if you are unable to make care decisions for yourself.
  • To be an organ donor if you are over 18 years old.
  • To have relief from pain.
  • To be free from all forms of abuse and harassment.
  • To have your health information protected.
