Saint Alphonsus pledges to provide care that respects the dignity of each person and have outlined our commitment in “The Pledge of Patient Rights and Responsibilities.” A copy of these rights and responsibilities is posted in each clinic.
You have the right to obtain copies of your medical records. We will ask you to complete a form to give us permission to release you or your child’s records. We require 7-10 days to process your request.
